Forms 1094-C and 1095-C will be required of employers with 50 or more full-time equivalent employees. For calendar year 2015, the forms must be filed by February 28, 2016, if paper filing, or March 31, 2016, if filing electronically. Many employers are evaluating software systems now that will track all of the information required for filing and systems to automate that process.
Information that will be needed includes name, social security number and specific months of employer-sponsored coverage for each covered employee, employee share of premium for lowest cost minimum value plan, total number of full-time employees in each calendar month, total number of employees for each calendar month, employer identifying information and a company contact for verification.
